Does the delete button just clear everything that is on the avatar, or does it delete items too?
I cant remember what that ribbon is behind my avatar, is there a way to take that off? Or even a search button for things that are currently on my avatar, so I can easily un-equip/equip things?

Can't help you with your question about the "Delete" button because I'm not sure myself.

The item behind your avatar is Flowing Spring Ribbon (it is in the Wings section). When you view the Flowing Spring Ribbon in your Wardrobe it'll be highlighted green (because you're wearing it), if you click the item it should unhighlight and be removed from your avatar, you then need to click "Save Loaded Outfit".

You can also remove the item from your Wardrobe by clicking the little red button with a - sign on it and it'll be transferred to your inventory.

No search button unfortunately.

Do you mean the Delete Outfit button at the top?

It just deletes a saved outfit you have, if you clicked "Create Outfit" with a name. ^^ It doesn't remove the items you have, it just deletes a saved design.

Your items that you have added to the wardrobe will be safe. They'll still be there after you click Delete Outfit. Hope that helps!
