There's not a specific timer. You have to beat the other lesser bosses of that type (In Mal's case the other Crazed Druids). There's a chance Mal will spawn after you defeat a Druid. It normally takes a few of them to get him.

The named dont have a time related spawn, you have to triger him again by fighting the unnamed bosses until you get the notification. From my personal experience your odds of spawning him are about 1/10 or so.
