in game... like. I'm disabled, how am I meant to get any new clothes if it's all..like gems

Gold is used for the majority of purchases in this game. For example, you buy pets with it, you make user transactions with it, and you can expand your stables with it, for starters. In addition, you can use gold to purchase diamonds, so it isn't necessary to spend real money on them.
I assume that you meant the clothing items in the wardrobe; while paying with diamonds is one option, the preferable currency is AP. You earn that while battling, and it's much easier to obtain than the alternative. You may also acquire avatar items in battle as well as purchase them with gold from the trade broker.
