Not sure if this is a bug or not but i was in the Enigmatic Grotto and i found a hole, dug in it and then my screen turned white and said my entire party had died and had to be revived. Not sure if this was intended to happen or not?

Ah. Yes. This happens as a triggered event. A mine blows up, if I remember correctly.
It's not intended to do what it does though with the blank page. That's a conflict with something that was added later on.
So, yes, the party dying is intended. It's a bad outcome to an event. But I'll have to adjust it so it doesn't respond the way it's responding.
