My guess would be that your pets point allocation and equipment are not updated.
With the recent changes to how much level points a pet gets per level, and how proficiency points are allocated, to be fully successful in a mission, your pet will need all it's points in that stat, and the right equipment boosting that stat. The equipment probably needs to be close to the pet's level, and of a high rarity as well.
From what you've posted, it seems you're using the same pet for different type of missions. That may have worked previously, but with the way things are set up now, you really need specialised pets for missions if you want to get the full rewards. Pets that are not specialised to a stat type have a chance of failing.
My level 60 pets (no equipment) I use for level 50 missions never had a problem before the change. After the change, they started failing missions, until I equipped them with epic/legendary level 50-60 equipment, then they gave me the full rewards again.

Emerald moon's equipment are level 5-15 and uncommon/rare, when she is level 30. I'll suggest upgrading the equipment to something closer to her level, and preferably at least epic.
Plus, she seems to be a health pet. As far as I know, all equipment types give the same amount of health, there is no type that specialises in boosting health. So you may need a better rarity to increase the difference needed.
