I've gotten a placeholder all times in a row. So my suggestion is that after you defeat a placeholder in a map position, there's less of a chance to get the placeholder in that same map position after the spawn time, and a higher chance to get the named. And after a named pops up in that map position, the placeholder / named chance resets back to the original.
