What is the best way to explain that you may be a victim of abuse to the police? I recently found out i have been abused via medical neglect and i need to tell some form of authority before my medical issues get worse.
Any advice is very appreciated.

Perhaps try to call 911 and explain your case to them. They'll probably handle the rest.

Call 911 and explain. Do you have any realitives or friends that you could tell and they could help you?
