It's just names and things you get for completing different tasks. Like the two titles I own I got from the trick or treating in the fall festival. Each title you get by doing a different thing, I'm not sure how you get a good majority of them though.

Most titles come from festival events. The only one I know of that does not is the one from the coliseum called "The Gladiator".(or something like that)
The fest titles currently available are:
-Trickster: Trick or Treat 500 times as a Visitor
-Candy Dispenser: Hand out 500 Pieces of Candy
-The Sweet Tooth: Create 5 Handfuls of Candy
-The Kind-Hearted: Hand out a Personal Item
They are all gotten from the trick-or-treating function.
