I see many players with titles under their names how do you get them?

They are usually prizes from festivals.
The next one is around Valentine's Day.

You'll have to work for your title for most cases.
Some you can get by participating in official forum games (like Fright Master)
Some you can get by doing enough of a certain action in a festival event/activity (like the Hallowed, the Sweet Tooth)
Some you can get by winning in the wheel raffle (like the Dreamer)
I think you can get titles from fighting in the arena as well. (Not sure since I don't fight there)
