She is captured and has never had one used on her. Yet when I go to use one it's the use button is grayed out. Why?

She already has one used on her.

edit: you can see, under where it says "This is a captured sylesti", there's an icon that looks like a mutator. It shows she has been mutated before

Yeah I figured out how to use it and that you can't from your inventory xD thanks anyways though I forgot to update the thread.
