It was a Vorkid Drone(I think) but throughout my entire battle, it showed this:
While obviously, it's not a big deal, I decided to go ahead and report it in case you have an issue with strange behaviors of enemies in the battle scenario. The name isn't reporting where they're attacking my Sylesti.
I believe it only happened in that battle, and when facing another Vorkid Drone, it seems to function fine. So, not sure. I cannot get the bug to recreate itself to show how I ran into it, sorry.

Okay, I've figured something out with it.
When you're attacking, do your pets attack first or second? And does it read that exact order in the battle information? At the end of a battle against 2 vorkid Drones, it happened again. But only at the very end. But it's reported at the top of the battle information.

The "'s Effect Blah" is because the enemy that placed that Debuff on your pet has since been defeated and the game doesn't know who the debuff belongs to.
It's something I've ignored for a long time now -_- I just haven't quite figured out how I want to deal with it. But as you said, it doesn't have any affect on anything.
As far as the order of battle, that is determined from each participant's Quickness Rating with a small random factor added in.
The Combat Log does indeed display exactly how the turn was calculated.
