... Illusions are optical. Therefore, they can't cause any pain to anything other then the eyes. They are not actually there, it's just a trick of the eyes. According to Eggman, the ruby messes with light and depth perception, and they are little more than a marage. How then can Eggman use something that's not there to wreck a city, fight the Resistance, etc? Pain goes beyond depth perception.
Unless....
...
There is a dark side to the Phantom Ruby.
It doesn't mess with just vision.
It messes directly with your nervous system.
That's how it can cause pain, and the illusions can be manipulated accordingly.
And that also explains why Sonic looks perfectly fine after that fisrt cut scene when he go KO'd. Infinite is the only one who actually hit him!
