So how do you make these kinds of things? If I could earn gold and diamonds off of something like this I totally would make a bunch of these. Do you have to have the items that you used to make the design?

Sylestia used to offer monthly sets, with ability to make custom items off it, mostly helds. Once you created a custom item, you can order additional copies for 100 diamonds.
Nowadays though, it has been discontinued. You can still get the clothing sets during Black Friday sales, but you can't create custom items.
