I've fought both grim reapers a few times, the barghests a few times, opened up every crate and pumpkin I can find, but I've only found 2 avatar items! Am I looking in the wrong places, or is the drop rate just really low?

The reapers are just placeholders for the actual named boss, a pair of banshees. Avatar items also have pretty low drop rates, so you'll have to do some farming to get items.
