I completed the quest in Esior's Fields where you have to defeat ten enemies, but I didn't get the armor. (I chose the red one, I forget what it's called.) Then I defeated a bandit in the fields and got a brittle assassin's relic and a level three xp orb, but it didn't give me the relic, just the orb. Both times my inventory was highlighted on the bar up at the top to show that something new was there, but neither item was actually there.

When viewing your Inventory, there's a separate tab called Equipment where Relics, Armors, and Accessories go.
You can switch to it at the top of the page.
