I'm guessing this is caused by the older code on the pre-TLG battle system, since it doesn't happen in any of the zones that use the newer system.
Basically, whenever a fainted pet is in the party whilst within the first two regions, any battle triggered will just...fail to fully recognize that pet isn't available to take action. It's certain that it can't do anything, but it's like it's just going "oh, there's a thing here, I guess. give it...something."
The only way to "restore" the action bar is by clicking an enemy, the pet that was supposed to have an action chosen, or by hitting a button related to the bar on the keyboard.
Not a massive issue as it's essentially just visual, but still odd.
